The outstanding results and “no downtime” convenience of most filler products have men and women- as young as those in their 20’s – seeking out these “real time” wrinkle erasers in greater numbers than ever before. From animal collagen and one’s own fat to bio-polymer hybrids and synthetics, the latest wave of injectable fillers is destined to expand the anti-aging repertoire of the dermatologic surgeon. Each substance brings subtle, distinctive benefits – some provide longer-lasting results, others allow for more precision with fine lines, and still others can gradually bring the face into better balance, creating an overall rejuvenated appearance. Moreover, skin specialists predict these exciting new substances could render face lifts pass.
1. Autologous Fat: Removes via a mini liposuction procedure from other parts of the body such as the thighs, buttocks or stomach, one’s own fat is used to improve deep wrinkles and contour the “hills and valleys” associated with the aging face. Also works well on certain scars and lip lines. There is no concern of allergic reactions.
2. Botulinum Toxin (Botox and Myobloc): While technically not a filler, very tiny amounts of botulinum toxin are injected into specific facial muscles to immobilize or relax the underlying cause of lines and wrinkles associated with expression, particularly forehead creases, furrows between the eyebrows, and crow’s feet.
3. Bovine Collagen (Zyderm and Zyplast): Having made its first appearance on the market nearly 20 years ago, bovine collagen is one of the most effective fillers available today. Still considered the gold standard for treatment of fine and moderate facial lines, improved production provide skin surgery specialists with refined animal collagen that flows more smoothly through the syringe. Thicker formulations are generally used for plumping lips, deeper lines and furrows. Bovine collagen can also be used on mild to severe scars. Two skin tests are required.
4. Human Collagen (Cosmoderm and Cosmoplast): Bioengineered from human collagen, the filler resembles the handing properties and longevity of bovine collagen. Injectable formulations are mixed with an anesthetic to improve patient comfort. Skin sensitivity tests are not required, enabling same day treatment.
5. Hyaluronic acid (Restylane, Perlane and Hylaform): Made from a basic element in connective skin tissue, hyaluronic acid gel, along with collagen, is the framework in which skin cells live. Extensive dermatologic experience in Europe, South America and Canada signifies great promise. Works well to reshape and smooth the surface of aging skin, particularly nasolabial folds, lips lines, smile lines and drooping corners of the mouth. The results appear to last longer than collagen and human tissue, between six months to a year. No skin tests required.
6. Hybrids (Artecoll): Micronized plastic spheres and bovine collagen are blended in the form of a permanent filler. The collagen component dissipates after injection, leaving the plastic spheres behind to shore up deep facial grooves, particularly around the mouth. Specialists advise injecting tiny amounts that can be built up over a few treatment sessions.
7. Liquid Silicone: For HIV/AIDS patients suffering from dramatic weight loss, liquid silicone has re-emerged as an effective option for filling out prematurely gaunt cheeks.
8. Synthetics (Radiance): A new breed of synthetic compounds that contain calcium hydroxyapatite, a substance used to replace missing bone. Designed to extend the permanence of collagen techniques, it works well for fleshy areas around the nose and mouth, deeper furrows and facial wasting related to HIV. Administration for cosmetic indications is presently considered an “off-label use.” |
